Registration Process
- Step 1: Pre-register your child via our online registration form.
- Step 2: After pre-registering online, please call to schedule your registration appointment. You can call the South Summit Elementary School front office at 435-783-4318.
- Step 3: Bring the following documents with you to your scheduled appointment at the South Summit Elementary School front office, 535 East 300 South, Kamas, Utah.
To enroll a student in South Summit Elementary, you must provide the following:
- Certified original birth certificate (must be original)
- Proof of residency (must be your permanent residence)
- Please bring one of the following:
- Utility bill or mail with your name and physical address on it.
- If you have a rental, please bring a copy of your rental agreement/lease.
- If you live with someone in our district, please bring the completed Affidavit of Residency form.
- Bring a copy of the child’s immunization record, or if you choose to file a personal exemption, please complete the online Immunization Exemptions form and bring it to our school. If your child’s immunizations have not yet been completed, please bring their current records. Once they are completed, notify the front office. All immunizations must be up to date before starting school.
- IEP and 504 (if applicable): If your student has an IEP or 504, please bring a copy of the plan when you register.
- If your child has any medical conditions the school needs to be aware of, please notify the school nurse via email.
